Insurance and Billing
We work with most insurance plans. Contact the office with any questions about yours. We will need an insurance ID number, toll-free phone number of the insurance company (we already have this for many companies) and the date of birth of the policy holder and all patients. In the event yours is not one of the plans we work with, we will provide you with the necessary forms after each visit filled out so you can get directly reimbursed by your insurance company. We accept checks, cash and Visa or Mastercard.
